Podhealth Foot & Ankle Clinic (ABN 97 588 128 079) collects the details you type into the "Or send a note" form and the home-visit request form on this website so we can reply to you and arrange care.
The note form asks for your name, email, phone and message. The home-visit form asks for your goal, name, phone, email, date of birth, funding type, approved provider, suburb and the name and number of a family member or carer. Please don't put urgent clinical information into either form. For urgent or life-threatening concerns, call 000.
Form submissions are received through our website host, Netlify, and are sent to the practice. We use them to contact you about your enquiry or home visit. We don't use them for anything unrelated to your enquiry.
